Dual tooth band setter

I got your band setter probably 5 / 6 years ago and never used it, the other day I pulled it out to put into service only to find out I don't have ( can't find ) the hardware package and instructions. could you help me out with this?
I went on your site and the one you offer now is nothing like the one I have.
If it can't be nailed or glued then screw it
