tree growth record keeping

I am trying to find an organized method for keeping records of the growth of selected and marked trees.

I understand that there are computer programs that can do this but since I have trouble working my TV remote and still use a track phone I am hoping that there are other fossils out there who have been doing this on paper who could help me. I have been using a write in the rain notebook to record diameters but am spending a lot of time flipping pages and also can't always find what numbers I have already used when starting to record new trees.

Are there any ready made books or forms for this or do any of you have a working system for doing this?
