Log buying forms

Hey so what do you guys have for a form to give to loggers for buying logs in?  Right now I have two sheets, both I made on MS Word and are nuttin fancy.  The one form is a tally sheet and just has a bunch of boxes, has my biz name and number at the top.  The other one is just a summary of the transaction.  It has my sawmill name number and address at the top.  Then it has sections with lines to write stuff on looks something like:


Date: ___
Bought:_________
Trucker: ______
Seller:_______
Total BDFT:_______
Total Paid:_________
Notes: ________




Kinda thinking about doing something more fancy.  Or even getting a print shop to make one with carbon copies so that I can rip them off a copy without having to fax/mail/email it over to them later.

I would think the trucker would be giving you a copy of something.
Any time I deliver logs, I have to give them a copy and bring one back, no such a thing as faxing, emailing or anything else later. Oh yea, and depending on who, a check comes back with me too.

My receipt looks like my chicken scratch on a dupilcate  book from DG or walmart. I usually pay for each load the same day with a check.

Is that the only form you give Peter?  Do you make a copy for them at the mill or mail it to em?

There is a #2 page, I get the top and the logger gets the bottom page.
Every log is recorded on it with lenth, topend,BF, grade, any deductions .

It might help to label the form you print as INVOICE, or SALES RECEIPT. Also you can ask your trucker if he writes out a blank BILL OF LADING for loads he hauls, stating what he is hauling, where it loaded at, and where it is going. And if he gets a printed SCALE TICKET (for weight). This would help for future record keeping.
