Received an Alert from our H&S group last week.
Our S.American group had an accident where a fellow was killed.
By the report, a mobile crane was set-up and somehow a ground support worker
moved in behind the crane to access an under-chassis tool-box to get some slings.
This fellow was in the operators blind spot and did not communicate verbally and/or visually
with the operator. Operator swung the crane and the weight stack came around.
I feel its not a waste of time to remind all the FF of the absolute importance
of confirmed communication with equipment operators (and use the 3 way rule when ever possible)
when you are on the ground in the equipment's area of influence.
Be careful out there.
Best
Dan
Apparently a cousin I barely knew was killed the same way.
